Please include:

•     Medical Records: Any recent progress notes or relevant patient history.
•     Diagnostic Results: Copies of lab work results, imaging reports, or other diagnostic test results.
•     Radiographs: Attach radiographs or other imaging if applicable.
•     Medications: A list of current medications, including dose and frequency.
