Steps to refer a patient:

• Fill out the Referral Form online​
  • We will accept faxed or emailed referral paperwork as well if you cannot complete the online form​.
• Email any radiographs to [email protected]
• Email or fax ALL PERTINENT labwork. (Fax to 502-901-7990)
• You may call or text (502) 901-7990 to discuss the case with a clinician or if you have any questions.  
  • You will likely have to leave a message to discuss a case, but we will return your call within 1 business day.  (Text messaging is usually faster)
• Have your Client Schedule their Appointment Online by visiting our website
• You also have the option to make the appointment online for your client, so everything is set-up even before they leave your building!​
  • ​They also can call or use the online portal to schedule the appointment themselves
